Chronic Disease Management

As our aim is to achieve optimal health for all patients, our doctors can manage and co-ordinate all aspects of Chronic Disease Management.

Our doctors and trained nurses are available to produce Care Plans, which are comprehensive and tailored to the individual’s problems or needs, ensuring an action plan for each.

Some Care Plan patients are eligible for referral to Allied Health providers at a subsidised cost. The Chronic Medical Conditions seen include Asthma, Chronic Bronchitis, Diabetes, Osteoporosis, Heart Disease, Renal Dysfunction, Stroke, Cancer and Obesity.
